Welcome to Hampstead Primary Care! To become a patient, please download, print, and complete the forms below. You can email the completed forms to office@hampsteadpcp.com or hand them to our front office staff. Be sure to read the first 2 pages in the entirity prior to filling out the form.
Please be aware that we do not prescribe long-term opioid pain medications or benzodiazepines. You are still welcome as a patient, but we will refer you to pain management or psychiatry for those specific medications.
At this time, we are not accepting new patients with Cigna or United Health Care as the primary insurance.
